Neura
the patient app
A voice first companion. Patients log migraines, medication and triggers by talking for two minutes a day and walk into the doctor with a report.
My roleOwned it end to end with one engineer: spec, conversation design, voice AI, QA and the store release.
The challenge
Headache diaries get abandoned within weeks; real world completion is around 39%. Visits run on memory.
What shipped
A realtime voice agent with tools for attacks, medication and diaries, insights with the quotes behind them, a clinician report, and consent first data handling.
What moved
Live on the App Store since 3 Sep 2026, built by two people, after clearing one store rejection
The voice flow
Consent, a two minute conversation, a tap on the head map, and a report the doctor can use.
- 01
The whole loop in five screens
Meet Neura, talk instead of typing, show where it hurts, see the pattern forming, and take a report to the doctor.
The voice flow, start to finish. Real App Store screens, demo persona. - 02
Meet Neura, consent first
Consent is versioned and blocking. Anyone who declines AI still gets a plain questionnaire.
Hello, I am Neura. Your data stays yours. - 03
Talk, do not type
Tap to talk, hold to talk, or type. Neura asks themed questions and captures answers quietly.
The day one baseline. Home, built around the daily check in. - 04
Show where it hurts
Tap every area on the head map, front or back. Neura asks about intensity and triggers in the same conversation, so a full record takes under a minute.
The head map: right temple and behind the right eye. Intensity. Early signals, phrased as signals. - 05
Medication and insights
Neura reads the medication cabinet and builds patterns from the patient's own logs.
Medication cabinet. Weekly insights. - 06
A report for the doctor
A clinician ready summary, plus gentle nudges and a recap that keep the habit going.
Doctor report. Demo persona. Nudge. Welcome back recap.
How it was evaluated
- Scripted beta scenarios with partner clinic staff; 21 issues tracked to closure
- Clinical and compliance review before the beta
- App Store review as an external gate: one rejection, fixed, then approved
Retention, diary completion and insight accuracy are not measured yet, so they are not claimed.
How it changed
- Apr to May 2026Insights, demo mode, guided tourDemos had to run without a backend
- JulHands free made visible; softer insight wordingEvery tester missed it; insights over claimed
- AugAI behind explicit consent, questionnaire fallbackApp Store rejection
- 3 SepPublic release
Technical choices
- Realtime speech to speech over WebRTC with a tool registry
- React Native and Expo, consent versioned on the server, personal data masked and audit logged
What I learned
- Voice first needs discoverability work.
- Consent is product design, not legal copy.
- Insights must under claim.
